Education Research/Scholarly Project Approval, Survey/Participation and Data Requests

USE THIS FORM TO:

  • Submit an MD Program Education Research/Scholarly Project Proposal to the Associate Dean for approval - including recruiting MD students
  • Request data from the MD Program

All MD Program stakeholders (students and faculty) undertaking MD Program related education research and scholarly projects must receive approval from the Associate Dean, MD Program. Similarly, any survey or study participation requests sent to MD students must receive approval from the Associate Dean, MD Program. The Office of Assessment and Evaluation (OAE) is facilitating these processes and all requests must be submitted via the webform below.

Please note, this form is only to request to recruit undergraduate MD students. Requests to recruit U of T postgraduate medical education fellows (‘medical residents’) should be sent to the Associate Dean, Post-MD Education within the Temerty Faculty of Medicine, at adpgme@utoronto.ca.
