Request data from the MD Program

All MD Program stakeholders (students and faculty) undertaking MD Program related education research and scholarly projects must receive approval from the Associate Dean, MD Program. Similarly, any survey or study participation requests sent to MD students must receive approval from the Associate Dean, MD Program. The Office of Assessment and Evaluation (OAE) is facilitating these processes and all requests must be submitted via the webform below.

Please provide us with details to support your request. If submitting a proposal for approval by the Associate Dean, MD Program please provide a brief summary of your research/scholarly study. For MD student survey and participation requests please provide a brief description of the study, the dates that you plan to collect data, and any other relevant information such as the survey link. For data requests please outline what data you are looking for, the objective of receiving the data and the timeline for receipt of the data.

Supporting documents

Please upload any relevant documents here. For submission of MD Program Education Research/Scholarly Project Proposals, please attach the project protocol and a copy of the data collection tools (REB is suggested if it has been obtained). For external survey and participation requests to MD program students, please attach the project protocol, the REB approval from your home institution and the data collection tools.
