Michelle Hart

Dr. Michelle Hart is an Assistant Professor in the Department of Family and Community Medicine, University of Toronto. Since 2009, Dr. Hart has been a staff physician at Baycrest Health Sciences Centre. Her current practice includes Complex Continuing Care, and Geriatric Rehab on the High Tolerance and Slow Stream Rehab Units. At Baycrest, Dr. Hart is the Chair of Medical Education, and the Education Coordinator for the PGY-1 and PGY-2 Family Medicine Residents. Her research interests include safe prescribing in the elderly, patient-oriented discharge planning, advanced care planning, and education program and evaluation. She has conceptualized and implemented the Baycrest Advanced Geriatric Life Support (BAGLS) manual and scenario-based training course, as well as research in BAGLS curriculum development, education needs assessment and outcomes.

Dr. Hart has received numerous teaching awards and is involved in teaching learners at all levels in undergraduate, post-graduate and continuing education of faculty. She has presented and taught workshops at many national conferences. Dr. Hart is a Graduate of Medicine at the University of Toronto where she also completed her residency in Family Medicine, and a PGY-3 Enhanced Skills Care of the Elderly Fellowship at the Department of Family and Community Medicine, University of Toronto. Dr. Hart has received her Certification of Added Competence in Care of the Elderly. She is a Fellow of the College of Family Physicians of Canada. She has a Masters in Science and Community Health through the Dalla Lana School of Public Health at University of Toronto in addition to certification through the Collaborative Program in Aging and Palliative Care at the Institute for Life Course and Aging, University of Toronto.
