Information for Elective Assessors

Elective supervisors can complete the Clinical Assessment form, once available, by completing the following steps: 

Step 1:   Go to http://medsis.utoronto.ca 

Step 2: Click on “Login to MedSIS” 

If you have forgotten your password, click on the "Forgot your password?" link, enter your email address and click “SUBMIT.” You will then receive an e-mail which indicates a PIN and password to login and complete the evaluations in MedSIS.  

Step 3: Once you have logged into MedSIS you can click on Student Evaluations on the left-hand menu. 

Step 4: The Clinical Assessment form you are required to complete for each elective student you supervised will be seen there.  

Step 5: Fill out the Clinical Assessment Form and click “SUBMIT.”  

If you did not supervise a placement for the student identified or should you experience technical difficulties while accessing the forms, please contact the MD Program Electives Office for assistance.

Elective supervisors can complete the Demonstrated Professional Behaviours Assessment form, once available, by completing the following steps:

Step 3: Once you have logged into MedSIS you can click on Student Evaluations on the left-hand menu. 

Step 4: The Demonstrated Professional Behaviours Assessment form you are required to complete for each elective student you supervised will be seen there.  

Step 5: Fill out the Demonstrated Professional Behaviours Assessment Form and click “SUBMIT.”  

If you did not supervise a placement for the student identified or should you experience technical difficulties while accessing the forms, please contact the MD Program Electives Office for assistance.  

Once an assessment form has been submitted, it requires assistance from the MD Program Electives Office to be “unlocked” for further editing. Please contact the MD Program Electives Office for assistance. 

If you have concerns about a learner's academic performance, such as clinical skills, you should address the concern with the learner as early as possible and provide specific, constructive feedback to support improvement during the elective. Early feedback gives learners an opportunity to reflect on and address concerns before the end of the placement. 

If the concern is significant or persists despite feedback, please contact the the Electives Course Director: Kimberley.kitto@utoronto.ca or MD Program Electives Office for guidance as soon as possible rather than waiting until the final clinical assessment. Early communication allows the MD Program to provide support to both the learner and supervisor and to determine whether additional follow-up is required.

If you have concerns about a learner's professional performance, such as communication or attendance, you should address the concern with the learner as early as possible and provide specific, constructive feedback to support improvement during the elective. Early feedback gives learners an opportunity to reflect on and address concerns before the end of the placement.

Most elective assessments can be completed in 5 to 10 minutes, particularly when supervisors have provided feedback to the learner throughout the placement and can readily summarize the learner's strengths and performance. Taking a few moments to provide specific, constructive comments helps ensure students receive meaningful feedback and supports accurate documentation of their achievement and progression.

The completed assessment becomes part of the student's academic record and may be accessed by individuals who have authorized responsibility for student assessment (i.e. Elective or Clerkship Directors), records management, and academic progression within the MD Program. Information from final clinical assessment forms may also be used in the production of the student's Medical Student Performance Record (MSPR).

If an elective assessment is not completed by the requested deadline, the preceptor will receive automated reminder emails every seven days until the evaluation is submitted. The number of reminders varies by learner year, with up to four reminders for Year 4 electives and up to three reminders for Year 3 electives. 

If an evaluation remains outstanding 14 days after the elective end date, the MD Program may notify appropriate department contacts, such as the: 

• Department UGME/Electives Lead 

• Department Chief 

• Medical Education Office Contact 

This escalation process helps ensure assessments are completed in a timely manner to support learner feedback, academic records, MSPR preparation, and progression decisions.  

The deadline for students to submit their MSPR is October 31. Narrative comments will no longer appear on the MSPR after that deadline.  

Tip: If you anticipate a delay, submitting the evaluation as soon as possible is appreciated, as students rely on timely assessments for feedback and documentation of their performance.

Elective assessments are an important component of a student's academic record. They provide learners with feedback on their clinical performance, professionalism, and areas for continued development, while also helping the MD Program monitor student progress and achievement of program expectations. 

Assessment information is used to: 

• Support learner growth through formative and summative feedback.  

• Document academic performance and successful completion of elective placements.  

• Inform academic progression decisions within the MD Program.  

• Contribute to the Medical Student Performance Record (MSPR), which is provided to residency programs through the Canadian Residency Matching Service (CaRMS).
