How Do I Disclose or Report?
Using the Disclosure Form
- The first step to either disclosing or reporting is to use the MD Program Disclosure Form. The form is an online tool that you can use to reach out and provide information to a designated MD Program leader regarding any behaviour experienced or witnessed that you perceive or suspect as being mistreatment.
- The designated MD Program leader will strive to contact you within 3 business days of receipt of an disclosure form to start a discussion regarding the event, possible next steps and supports.
You have the option submit a Disclosure Form anonymously (i.e. without the requirement that your provide their identity), with the understanding that doing so is subject to certain limitations.
Contacting a MD Program Leader
Although students are encouraged to use the MD Program’s Disclosure Form, you may also contact designated program leaders by email. Email communications should be clearly dated and labelled “Confidential report for the attention of Dr.” to ensure priority review.

Alternate Forms of Feedback
Course Evaluation Forms
Although we recommend using one of the above methods for disclosure, you also have the option of providing information regarding experienced or witnessed mistreatment on course and teacher evaluation forms. While every effort is made to review evaluation forms in a timely manner, you should not assume that course or teacher evaluations will result in action in the same manner as a report of learner mistreatment.
